Enrollment Forms

Most families will need all forms. However, the Individual Infant Sleeping Plan only applies to infants under 12 months, and the Blood Glucose Testing Consent only applies if your child has diabetes or a related condition. We'll confirm exactly which forms are required for your child during your enrollment visit.

Yes, most PDF forms can be completed digitally using Adobe Acrobat Reader (free) or any PDF editor. You can then print and sign, or bring a signed printed copy. Please ensure all signatures are original (handwritten) unless we advise otherwise.

Along with the completed forms, please bring: your child's immunisation records, a copy of their birth certificate, and any doctor's notes regarding specific health conditions or medications. We'll review everything together at your first visit.

Most forms are completed once at enrollment. Immunisation records should be updated whenever your child receives new vaccines. Emergency contact information should be updated any time it changes. We'll remind you when renewals are due.

These are official State of California forms required by the Department of Social Services and Department of Public Health for all licensed family childcare facilities. They are not specific to Happy Feet — they are mandatory statewide documents.

Yes, you're welcome to scan and email completed forms to happyft.mh@gmail.com ahead of your start date. We'll review them in advance so your first day runs smoothly. Original signed copies are still required to be brought in person.
