Forms Questions Answered

Do I need ALL the forms on this page?

Most families will need all forms. However, the Individual Infant Sleeping Plan only applies to infants under 12 months, and the Blood Glucose/Nebulizer Consents only apply to specific medical needs. We'll confirm exactly which forms are required for your child during your enrollment visit.

Can I fill the forms digitally instead of printing them?

Yes, most PDF forms can be completed digitally using Adobe Acrobat Reader (free) or any PDF editor. You can then print and sign, or bring a signed printed copy. Please ensure all signatures are original (handwritten) unless we advise otherwise.

What documents should I bring alongside the forms?

Along with the completed forms, please bring: your child's immunisation records, a copy of their birth certificate, and any doctor's notes regarding specific health conditions or medications. We'll review everything together at your first visit.

Are these forms specific to Happy Feet or general California forms?

These are official State of California forms required by the Department of Social Services and Department of Public Health for all licensed family childcare facilities. They are not specific to Happy Feet — they are mandatory statewide documents.
